Add a signature to a form

In the side menu, click Forms, then Management. Find the form, click its ⋮ menu and choose Edit.

Click Add field. A new question is added at the end of the form and its settings open.

In Field Label, say who is signing or what they're agreeing to — for example, who signs off the check.

Click Type and search for signature (or sign, consent, agree), then choose Signature, under Upload & sign. The Field Preview shows the signature box. Under Signature Configuration you can change the box's size, pen colour and thickness, and whether signers can clear and redo it.

Tick Required if the form mustn't be sent without a signature, then click Done.

Click Save Form. Done only closes the question; the form isn't saved until you click Save Form.

When someone fills in the form, the signature question appears where you put it.

Type is selected to start with: the signer types their name and it's shown in a handwriting style. Draw lets them sign with a mouse, or with a finger on a touch screen.

The signer clicks Accept to keep the signature; until then the box reads Signature ready - click Accept to save. Once saved, Replace Signature lets them sign again.
