When your organisation works to a care or quality framework, Read + Respond can score form responses against it. Each area of the framework gets a suggested score from −2 (Significant Decline) to +2 (Significant Progress), with the evidence from the note that it's based on. A person then reviews the suggestions and completes the assessment.
Frameworks are set up by an administrator in Settings › Frameworks: import or create the framework, map its areas to your forms, and activate it. Responses to the mapped forms are then scored as they come in.
Step 1
Find the response
In the side menu, click Forms, then Management, then the Form Responses tab. Click the response to open it.
Step 2
Open the scorecard
Click Care Assessment Scorecard. The button only appears once the response has been scored. When a submitted note is scored, a Review Score Card task is also created, so the review can be picked up from the task list.
Step 3
Read the suggested scores
The assessment is grouped by the framework's domains. Each area shows its score on a five-point strip, with its meaning beside it, and AI until someone confirms or changes it. Some frameworks number the points 1 to 5 instead of −2 to +2.

Check the evidence
Click Show evidence to see what in the note the score is based on. You can edit the evidence as well as the score.

Change a score
If a score doesn't fit, click the one that does. The card is marked Edited, and the undo button puts the suggested score back.
Step 6
Confirm a score
Click the tick on a card to accept its suggested score; it's then marked Confirmed.
Step 7
Confirm the rest
Confirm All accepts every suggested score you haven't changed or confirmed yet. Check them first — confirming says you agree with each one.
Step 8
Save and complete
Click Save & Complete. A score of −2 or +2 needs evidence; you'll be asked to add it before the assessment saves. Discard Changes throws away what you've changed on this visit.
Step 9
Once it's complete
The assessment is marked Completed and your scores are saved. A completed assessment isn't overwritten if the response is scored again.
Step 10
Regenerating scores
While an assessment is still pending, the person who submitted the response can click Regenerate to re-score it from the current form content. Scores that have been edited or confirmed are kept; only suggested scores are replaced.
