Turn a paper form into a digital one

Upload a photo or scan of a paper form, or an existing document, and AI drafts a digital form for you to check and edit.

If your organisation already has paper forms, you don't have to rebuild them question by question. Take a photo or scan of the form, or use the Word or PDF file it was printed from, and AI reads it and drafts a digital version in the form builder.

The draft is a starting point: always check it against the original before you use it. To describe a form in your own words instead, see Generate a form with AI, which uses From Prompt.

  1. Step 1

    Open Forms Management

    In the side menu, click Forms, then Management.

  2. Step 2

    Choose From Document

    Click Create Form, then From Document. The Generate Form from Document window opens.

  3. Step 3

    Check what it accepts

    The Document box takes a PNG or JPG photo, a PDF, a Word document or a text file, up to 10MB. A clear, straight-on photo in good light reads best.

  4. Step 4

    Upload the form

    Click the box and choose the file, or drag it onto the box. It turns green and shows the file's name and size; the × removes it if you picked the wrong one.

  5. Step 5

    Leave the title to AI, or set your own

    Form Title and Form Description are optional. Leave them empty and AI suggests them from the document, or type your own.

  6. Step 6

    Generate the form

    Click Generate Form. The window closes and the form is drafted in the background, so you can carry on with other work.

  7. Step 7

    Watch the banner

    A banner at the top of Forms Management shows the file's progress — reading the document, then building the form — and changes to Form ready to use when it's done.

  8. Step 8

    Open the draft

    Click Open form on the banner. The new form opens in the form builder.

  9. Step 9

    Check it against the paper form

    Go through each question and compare it with the original. Fix anything that was read wrongly — a misspelled label, a question that should be a dropdown, a field that should be required — then save it like any other form.
